Claim: Resident rehabbing after femur repair surgery was dropped by nursing staff during a transfer, resulting in surgical plates shifting. After a second repair surgery, continued issues with surgical site infection, resulting in leg amputation and death.

Skilled Nursing Facility – Dallas, TX
Claim: Dementia resident suffered at least 11 falls in a two year period, resulting in several injuries and hospitalizations and overall decline of health.

Claim: Facility allowed stroke victim to develop severe stage 4 sacral pressure ulcer and pressure injuries to ankle and knee, resulting in infection, hospitalization, and ongoing wound care.

Skilled Nursing Facility – Spring, TX
Claim: Long-term care resident who required assistance with all ADLs was left unattended by nursing staff while being bathed. Resident fell out of bed, fracturing several bones. Facility delayed transfer to hospital until next day, where resident died due to injuries from the fall.

Claim: Long-term care resident with dementia developed severe stage 4 sacral pressure ulcer, resulting in hospitalization and aggressive wound care.

Skilled Nursing Facility – Trinity, TX
Claim: Resident, who was in facility for rehabilitation after back surgery, developed multiple severe stage 4 pressure ulcers. Facility also failed to monitor and change catheter on a consistent basis, leading to internal bleeding and resident’s death.
